Community Connected Care: A Community-Based Approach to Address Health-Related Social Needs and Improve Health

Healthier communities lead to healthier people. When health plans, businesses, and community organizations work together to address social drivers of health, better outcomes are the result. That’s the goal of our Community Connected Care (CCC) program. We created CCC to assess the health-related social needs of our health plan members and connect them to community-based organizations (CBOs) that can get them to resources and services to improve their whole health. The CCC program integrates local partners, data, insights, member engagement, and solutions to create a plan to address whole health. Member Eduardo Muñoz was introduced to the Partners in Care Foundation (PICF), a CBO that helped address his health-related social needs, which included a language barrier. With Spanish-speaking care providers and an array of resources and services aligned with his needs, Muñoz was better able to manage his health.
