Accessible Housing, Nutritious Food Can Improve Health Outcomes
Health-related social needs directly influence everyone’s health at various times in their lives. Unstable or unsafe housing, lack of transportation, and poor access to food can lead to higher rates of depression, emergency room visits, and lower quality of life. This was Helen’s experience; the 61-year-old member of an Elevance Health-affiliated plan sought treatment in the emergency room for multiple physical conditions more than 30 times in a year.
A health plan case manager, Hannah, reached out to Helen to offer support for Helen’s physical, behavioral and social health needs. Hannah connected Helen with supportive community resources and Helen received a wheelchair, a bed, accessibility and safety-related home improvements, and better access to more nutritious food. Helen and Hannah even attended medical appointments together, which provided Helen additional comfort and improved communication so the care provider could address Helen’s needs.
