A Whole-Health Approach Reduces Emergency Room Use, Improves Outcomes
People who frequently visit the emergency room often need additional support. Chavon Haley was no exception. Her health plan’s data showed that Haley — a young mother who is blind and has diabetes, kidney disease, and was experiencing homelessness — sought treatment in the emergency room almost weekly. That changed when Kidist Lemi, a registered nurse at Chavon’s Elevance Health-affiliated health plan, got involved. Using a whole-health approach, Lemi partnered with Haley to address Haley’s health-related social needs over the past two years. She connected Haley with ongoing healthcare as well as with additional resources, such as transportation. Haley shared how the additional support improved her life, from helping her attend doctors' and dialysis appointments to securing stable housing.
