Our Dedicated Family Advocates (DFA) are supported by state-of-the-art technology that allows them to connect members to programs in real-time, locate specialists, explore resources, and determine next steps. Advocates take a person-centered approach, focusing on understanding the member and their family with a view to identifying "silent" needs and risks and proactively working to remove barriers to health. Our members at highest risk for acute and chronic care needs are also assigned a dedicated Clinical Advocate who works one-on-one with members.
Because our service and clinical platforms are integrated, a DFA is alerted as soon as an unexpected life event occurs, and they can monitor items such as referrals to ensure the member has no "healthcare homework." They can also quickly draw on community connections to respond to needs beyond case management and health, such as transportation, grocery delivery, and childcare. Our DFAs are supported by a team of multi-disciplinary service and clinical experts who assist with complex issue resolution, serve as a rapid response team for member issues, and provide real-time data analytics identifying health risks.
